Complex dental and implant-prosthetic reconstructions present clinicians with the challenge of defining adequate vertical and horizontal relations. This is particularly crucial in cases where information about the original, natural function is lost due to tooth loss.
During rehabilitation, various parameters are considered: aesthetic, phonetic, and physiological aspects related to masticatory muscles, temporomandibular joint position, as well as lip and tongue positions. Determining the available space for teeth is essential for optimal tooth positioning. Therefore, in complex rehabilitation cases, we employ techniques proven successful in complete dentures, adapted to natural tooth pillars and implants. Knowledge of registration techniques and tactile sensitivity of teeth and implants is integrated into occlusal surface design. The choice of static and dynamic occlusal concepts is not only physiologically relevant but also crucial for avoiding technical complications, such as reducing the risk of wear on occlusal surfaces and fractures/chippings of incisal edges.
The presentation will illustrate current research knowledge on occlusal reconstruction, covering biological and technical aspects, and will exemplify practical, clinical implementation within the University of Bern's concept.
